I hereby give Vernon Denture Clinic Ltd. (VDC) permission to use the information provided in this Review (the “Review”) for any purposes in connection with promoting VDC and its activities (the “Purposes”), which may include advertising, promotion, and marketing. As needed, VDC may edit the Review to correct spelling, grammar, and punctuation, and may make other modifications to clarify unclear original statements, protect individual privacies, or otherwise ensure the Review is written suitably for VDC purposes; in this case, every effort will be made to preserve as much of the wording and spirit of the original Review as possible.

I understand that my personal information, including my Review information, is being collected pursuant to section 26 of the Freedom of Information and Protection of Privacy Act, R.S.B.C. 1996, c. 165, for the Purposes. I consent to my first name, last initial, and city and province of residence being displayed in connection with the appearance of my Review.
